Human Resources General Information for EmployeesThe following list includes many frequently used DAS-HRE forms in one central location for the user's convenience. These are generally forms that originate with the employee. Several of these forms are also found in other locations on the DAS-HRE website. The forms on this site can be used in one of two ways. They can be completed online and saved to your own computer, or they can be printed and completed by hand. The forms that are in PDF format can only be completed by printing the blank form and completing it by hand. None of the forms is set up to update an HRE database.
